If the result of your physician visit is to schedule an evaluation for therapy, ask your physician to refer you to the Rehab Center. Once we receive the referral, we will contact you to set up your

admission appointment and evaluation.

When we contact you for an appointment, we will ask you for 3rd payor (insurance, Medicare, Medicaid, Tricare, etc.) information. We will then contact your carrier to determine benefits and to help determine your financial responsibility. For those who qualify, sponsorships are available in most of our programs. If you wish, we can discuss these opportunities during your appointment.

After we receive a doctor's referral, we will get 3rd payor (insurance or medicaid) information and contact them regarding benefits. We will then let the patient know their coverage and their responsibility for any services.
