How to get started with therapy:
- Get a prescription/referral from your physician (this needs to have at a minimum the following;)
- Patient's name
- Address
- Phone number
- Date of birth
- Diagnosis
- Services being referred for
- Physician's signature
- Date of physician's signature
- Your physician can either fax the information to 940-767-3715, or you can call and schedule an appointment at 940-322-0771, ask for Admissions
- To help speed up the admissions process please complete an adult case history form or a child's case history form. Please bring the completed form with you when you come in to be admitted. Adults are considered anyone 17 years of age or older.
- For Children ages 3 and under, go to the Early Childhood Intervention page for admission information.
Sponsorship:
Sponsorship is offered to clients who have a financial need. If you feel that you have a financial need, you can ask the Admissions Counselor to complete a Sponsorship application. Our sliding fee scale is based on current federal poverty levels and family size. Applying for sponsorship is not a guarantee that you will receive financial assistance with your services.

You are encouraged to contact your insurance company by using the phone number for member services on the back of your insurance card to verify what your benefits for our services might be. If you have any questions regarding our role in benefit verification and/or authorization / pre-authorization please ask the Admissions Counselor when you call to schedule your appointment.
If you have questions about your bill, please call 940-322-0771 and ask for the billing department.
MEDICAL RECORDS
If you would like to request a copy of your records for yourself or to be sent to another facility, healthcare practitioner or insurance company, please complete the attached records release. Please make sure this is thoroughly completed and you may fax it to 940-766-4943.
